Job Description
Under the direction of the Chief Medical Officer, the Clinical Pharmacist plays an essential role in delivering safe, evidence-based, and patient-centered pharmaceutical care within an ambulatory care setting. The Clinical Pharmacist collaborates with the interdisciplinary healthcare team to manage drug therapy, optimize treatment outcomes, enhance patient education, and support quality improvement initiatives aligned with organizational goals.

Compensation: $71.04 - $81.40 per hour / $147,758.70 - $169,312.32, depending on experience, with a signing bonus available!

Responsibilities:
- Conducts patient medication history inquiries upon referral from physicians or other medical personnel.
- Based on a diagnosis, synthesizes a treatment plan that may include drug therapy and lifestyle modifications, and supports care transitions across settings.
- Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, subtherapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medications without indication.
- Directly supervises Pharmacy Technician Coordinators, including general responsibilities related to hiring, training, scheduling, coaching, and providing day-to-day support.
- Under protocol, initiates, adjusts, monitors, and recommends changes to drug therapy based on patient interviews, objective monitoring parameters, drug profiles, and the medical record.
- Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related tests.
- Monitors patients to evaluate and optimize outcomes, drug utilization, and compliance, while minimizing adverse drug reactions and eliminating unnecessary or duplicate therapies.
- Provides patient education regarding their disease state, including expected outcomes, side effects, proper compliance, and special instructions.
- Ensures ongoing performance improvement of pharmaceutical care services.
- Provides functional guidance to the care management team, analyzes and resolves issues to promote high-quality and efficient workflows that improve health outcomes.
- Monitors and analyzes clinical quality measures (e.g., HEDIS) to support high performance across all tracked metrics.
- Educates providers and staff through presentations, newsletters, utilization reviews, and one-on-one consultations.
- Performs other duties as assigned.
- Minimum of two (2) years of experience as a practicing pharmacist required.
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited college of pharmacy.
- Active California pharmacist license or FPGEC certification through the National Association of Boards of Pharmacy.
- Board certification in ambulatory care (BCACP) or completion of a pharmacy practice residency preferred.
- Current Basic Life Support (BLS) certification required.
Qualifications:
- Excellent communication skills.
- Ability to travel to designated locations as directed by supervisor.
- Ability to deliver care in both in person and virtual model.
- Bilingual in Spanish is preferred but not required.
- Clinical knowledge and educational background sufficient to function as a clinical pharmacist.
- Strong oral communication skills to provide clear, concise instructions to patients and families, consult with medical staff, and deliver program recommendations.
- Effective reading and writing skills in English to review medical charts and documentation, participate in research, and follow written instructions.
- Demonstrated courtesy, responsiveness, and professionalism with patients, colleagues, and other departments.
- Commitment to customer service by assisting internal and external stakeholders in a helpful and cooperative manner.
