Job Title: Primary Care Nurse Practitioner or Physician Assistant
Location: Hartford, CT
Job Type: Full-time
Salary: $100,000.00 - $165,000.00 per year
About Us:
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our clinic in Hartford, CT. Our Advanced Practitioners provide comprehensive primary healthcare, including health promotion and disease prevention activities, in collaboration with department physicians. We are committed to providing high-quality, culturally competent care to a diverse patient population in a collaborative environment.
Responsibilities:
- Provide comprehensive primary health care to the adult patient population.
- Conduct health promotion and disease prevention activities.
- Collaborate with department physicians on specialized care.
- Refer patients to consultants for diagnosis confirmation and treatment as appropriate.
- Take medical histories, assess medical conditions, and interpret findings.
- Maintain timely, thorough, and accurate written clinical documentation.
- Make evidence-based clinical decisions and comply with quality measures.
- Utilize electronic health records and other computer systems.
- Perform other duties as assigned.

Qualifications:
- Graduate of an accredited school of nursing and successful completion of an approved nurse practitioner program (for NP candidates) OR
- Deep knowledge of physician assistant theory and practice (for PA candidates).
- Valid State of Connecticut APRN or PA licensure (Preferred), or willingness to obtain a CT license.
- DEA and state narcotic registration, or eligibility to obtain.
- Proven record of providing high-quality, comprehensive, professional clinical care according to established standards, protocols, and practices.
- Ability to provide culturally competent care to a diverse patient population.
- Track record of effective communication
- Comfortable using electronic health records and other computer systems.
- 1-2 years' experience working with the adult population
