Case Manager

Location: Berkeley, CA
Hours: Monday to Friday, 9 AM–5 PM
Compensation: $100,000–$120,000 annually

Join our team as a Case Manager and help individuals make meaningful progress through coordinated care and ongoing support. This role is based fully in office and offers consistent daytime hours. You’ll assess client needs, develop service plans, and ensure appropriate connections to healthcare, housing, counseling, or other supportive programs—advocating for stability every step of the way.

Your Role Will Include:

  • Developing and implementing client care strategies
  • Acting as the main contact for assigned cases
  • Coordinating services, resources, and follow-up appointments
  • Documenting all activity clearly and promptly
  • Collaborating with care teams and community partners
  • Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (Master’s preferred)
  • At least 2 years of case management or client coordination experience
  • Strong communication and organization skills
  • Familiarity with local support systems and services
  • Licensure (e.g., LCSW, LMSW, RN) is a plus
