Responsibilities
Pharmacist – St. Bernardine Medical Center

St. Bernardine Medical Center is seeking a skilled and detail-oriented Pharmacist to provide comprehensive pharmaceutical care across inpatient services. In this vital role, you will ensure safe medication practices, participate in performance improvement initiatives, and serve as a trusted resource for clinical staff and providers.
Key Responsibilities:
- Review, process, and dispense medications and IV therapies with precision and in compliance with all hospital policies and regulatory guidelines
- Provide direct clinical consultation and medication therapy recommendations to physicians and nursing teams
- Supervise and support pharmacy technicians in compounding, repackaging, and medication distribution
- Monitor patient medication profiles for drug interactions, allergies, and contraindications
- Perform pharmacokinetic calculations and adjust dosages as appropriate for patient age, weight, and condition
- Support formulary compliance, therapeutic interchange, and clinical pharmacy initiatives
- Participate in department performance improvement projects, medication use evaluations, and adverse drug reaction monitoring
- Ensure accurate and timely documentation within the EMR and pharmacy systems
- Maintain compliance with controlled substance protocols, regulatory standards, and documentation requirements

Qualifications
- 1-2 years experience in Hospital Pharmacy, or completion of a Pharmacy Residency program preferred
- Experience in unit-dosing, IV admixtures, pharmacokinetics and TPN desire
- Graduate of a School of Pharmacy, required Pharm D. desirable
- Basic knowledge of computers desirable
- California Pharmacist License
