Responsibilities
Under the direction and supervision of the Director of Nursing, the MDS Coordinator is responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS), and for tracking, and transmitting all data including the RUGS to the Department of Health Services. The process includes the coordination of appropriate participating health professionals (interdisciplinary team) for the purpose of conducting initial and periodical comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his / her highest practicable level of physical and mental and psychosocial functioning.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
1. 2 years of experience in Skilled Nursing Facility working with MDSs
2. Current state RN License upon hire and maintain current.

The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
