About the Position
The Community Health Worker (CHW) will be assigned to support a program providing post-partum home visiting support services to women in 1-2 counties across a 5-county area (Washington, Bolivar, Carroll, Humphreys, and/or Scott). The CHW will organize activities to provide face to face educational programming regarding postpartum health and well-being to small groups within their catchment areas. Upon referral from a nurse, the CHW will have direct contact with the postpartum patient to complete the required home visits and any required or recommended assessment, provide services to the patient and families as indicated by a postpartum plan of care, address barriers and needs within their scope, provide health education and supportive counseling, and/or participate in case conferencing providing guidance and recommendations for the continued postpartum care of the patient.

Strong communication and interpersonal skills, ability to establish rapport with families and empower them by building on their strengths, strong organizational and record-keeping skills, public speaking, ability to work independently, familiarity with issues surrounding family poverty, commitment to working with diverse people and families, provide services in a culturally appropriate/sensitive manner, maintain professional boundaries and confidentiality, demonstrate ability to work as an effective team member. Must be able to travel across a 1-2 county catchment area and provide services in the homes of participants.
Preferred Qualifications
Typically requires a bachelor’s degree and 0-2 years of experience. Appointing authority may require social worker’s license.
